Course Description:

GERD, Barrett’s Esophagus and GI Screenings

The landscape of gastrointestinal care is shifting fundamentally, driven by newly released consensus criteria, next-generation acid-suppressive therapies, and evolving screening strategies.

Dr. Jason Farrell will break down the essential updates from the 2024 Lyon 2.0 Consensus, which establishes new, objective standard parameters for diagnosing GERD based on physiology and advanced monitoring metrics rather than symptom response alone. The presentation will critically evaluate the comparative pharmacology of traditional PPIs against Vonoprazan (PCAB) - the first novel acid-suppressive mechanism introduced in decades - and outline precise clinical criteria for matching refractory patients to endoscopic or surgical anti-reflux interventions. Finally, the session will review the latest risk-stratified AGA 2025 surveillance intervals for Barrett’s Esophagus and dissect the clinical realities of emerging blood-based colorectal cancer screening tests. Attendees will leave equipped with evidence-based principles ready for immediate clinical execution.

Key Learning Objectives:

  • Implement Lyon 2.0 Diagnostic Criteria: Accurately classify patients into conclusive, borderline, or excluded GERD phenotypes using objective physiological metrics (such as Acid Exposure Time and MNBI) and strategically determine when to perform ambulatory reflux testing on vs. off PPI therapy.
  • Optimize Acid-Suppressive and Anti-Reflux Therapies: Evaluate the clinical utility and distinct pharmacokinetic advantages of Potassium-Competitive Acid Blockers (PCABs) compared to traditional PPIs, and apply current guideline criteria to properly identify candidates for surgical vs. endoscopic anti-reflux interventions.
  • Execute Risk-Stratified Screening and Surveillance: Apply the updated 2025 segment-based surveillance intervals for non-dysplastic and dysplastic Barrett’s Esophagus, and critically counsel average-risk individuals on the appropriate, "last resort" utility of blood-based colorectal cancer screenings compared to standard colonoscopy workflows.
